The following survey contains 20 closed questions that evaluate people eating habits and possibly be use for future medical and nutrition investigation. Before filling out this questionnaire, it is important that you understand why the research is being done and what it will involve. This leads us in the positions of welcome and respect for people of all ages, gender, race, ethnicity, religion, disability, sexual orientation, sexual identity, education, national origin, or any other distinguishing characteristic or trait. Participant data will be kept confidential except in cases where the researcher is legally obligated to report specific incidents or details. These incidents include, but may not be limited to, incidents of abuse and suicide risk. Your participation in this study is voluntary.
